It depends on the medication they take or the NuvaRing they insert.
It can be every 3 weeks, and it can be every 4 months. The body follows the instructions of science. Sometimes they do it one way, other times differently, as they wish.
Are all the calculations of menstrual cycles relevant for them at all?
It is known that they will get their period when they do what they need to do in order to get it, and not before and not after.
And even if there are irregularities, that is not connected, not even remotely, to the menstrual-cycle calculations established in Jewish law.
It’s just random for medical reasons, and all the calculations are irrelevant.
So are they exempt from all these calculations?

I think that if there is a woman who sets the time of her period for herself through medication (assuming this is reliable and there are no unexpected periods), she is exempt from the laws of menstrual-cycle calculations.
