Second covering, prayer garment, and Brahms
Peace to the rabbi
Is there an obligation to pray and bless the food?
To put on a hat (or other second covering)
And outerwear
And is there a difference between Sephardim and Ashkenazim?
(And even Lithuanian Ashkenazim do not adhere to this in Brahms, for example)
Or does it depend on the place of worship?

I don't think so. And this has nothing to do with Sephardim or Ashkenazim, but rather with local custom. The issue is respectable clothing, and what is respectable changes from place to place and from time to time.
