Washing hands?

When is it obligatory and when is it not? When does reciting the blessings delay Torah study and prayer? Is it purification from impurity or just an additional purification? If one accidentally blesses without a kippah under the open sky, does one repeat the blessing? Because when there is a ceiling, I heard that one does not repeat it because there is an opinion that the kippah is considered a head covering.
