I suffer from diarrhea due to a medical condition, which means I need to wipe many times during the day, and sometimes that creates a situation where blood comes out, and with each additional wipe a little more blood comes out. The problem is that many times on the Sabbath I end up still not clean and needing more wiping, and of course if I stop in order not to cause a wound, there will be a problem saying sacred things, praying, etc. What can be done in such a situation?

Practically speaking, there is a washer (bidet) that it would be worthwhile for you to use.
As for the fundamental question: as long as it is not certain that your wiping will draw blood, this is not an inevitable result, and it is considered an unintended act. And even if it is certain, it is an inevitable result that is not beneficial to you, and in fact it is destructive, since you do not need the blood to come out. Because of human dignity, in my opinion it is permitted.
