I filled out a pruzbul on a website where they asked only for my name and ID number, and that was it—no phone number, email address, home address, or anything else.
Is that enough?
And if not, should I fill it out again on another site?

It is valid. On ordinary pruzbul documents, traditionally only the name appeared, even without an ID number. But I would not rely on a document and the execution of a pruzbul in a place that I do not know. You can go through recognized organizations and do it through them.

In my opinion, no. The intention is that a legally valid acquisition was carried out (apparently by your agents).

It’s a large website for halakhic questions and answers that gives the option of submitting a pruzbul through them online. It’s just that under the details it only asks for first name, last name, and ID number. So that’s okay?
