I heard from a pressured halakhic decisor that there were communities whose tradition was that an external wiping was sufficient and there was no need to insert the cloth, and somehow in recent years this practice has been fading away, especially among younger women.
If I understood him correctly, this is the view of seven medieval authorities (Rishonim), and perhaps this is also how the Noda B'Yehuda rules, and mainly this was the Jewish law practiced in those communities. It's a shame, because today many neglect the matter altogether since it is painful, injurious, makes it hard to become pure, humiliating, and women do not feel that this is God's will.
Seemingly, it would be better for them to return to their mothers' custom rather than neglect the matter entirely.
